We share your concerns regarding the rising cost of healthcare. The expense of billing adds to your cost of medical care and, therefore we ask for your payment of co-pays, deductibles and non-covered services at the time services are rendered. Additionally patients with commercial insurance will be required to pay in full at the time of service. We accept cash, checks, Visa and Mastercard.
As a courtesy to our patients, we will file insurance claims for those insurances with which we participate. Please remember, any amount not covered by insurance is ultimately the patient's responsibility. If you have commercial insurance or an insurance that we do not participate with, we will provide you with the appropriate paperwork to file claims.
We participate with Medicare and are contracted with many Managed Care insurance companies. We recommend you check with your insurance company to make sure that we are on their preferred providers list or that you have "out-of-network" benefits.
Please have your insurance identifications card with you at each visit and make us aware of any changes in your coverage. Any changes in name, marital status, address, or phone number need to be given to us at your earliest convenience in order to keep your records current and accurate.
Laboratory and radiology studies are billed by the facility performing the tests. Please notify us of the particular laboratory and radiology services that are contracted by your insurance plan.
